Medical Records Request

Whether you need to get your child’s record of immunizations to provide to their school or your child’s full medical history to give to a new family doctor, you can request the records you need.

To request your child’s medical records from KIDZ Medical Services, please contact one of our customer service representatives who will gladly assist you over the phone or visit your local KIDZ Medical Services location.

Our customer service representatives are available Monday- Thursday from 8:30 am to 4:30 pm and Friday's from 8:00am to 2:00pm at 305-661-1515, ext. 235.
